The New York State Department of Health (DOH) Traumatic Brain Injury (TBI) waiver program provides services to persons with a TBI. The purpose of the program is to help persons with a TBI live in the community setting of their choice.

Personal Care Services include bathing, toileting, feeding, dressing, grooming, light housekeeping, shopping, meal preparations, escort to appointments. This program is available through Medicaid, private payment and some health insurers.

your family to discuss your needs and develop a home care plan to meet your goals. The Nursing Supervisor will recommend the appropriate level of health care personnel to meet your health care needs. You, your family, and your doctor will decide what hours of care are needed. Innovative Care will select staff members to provide for your care. Innovative Care accepts private pay accounts as well as third party commercial insurance assignments. Innovative Care will bill your insurance company for your services, which means the services under Medicaid are at no cost to you!
